We’re a local team of clinicians, nurses, and support staff dedicated to making great care simple. From easy scheduling to clear billing, we aim to remove the stress so you can focus on feeling better.
We started this practice to bring the best parts of medicine closer to home—appointments when you need them, clinicians who have time to listen, and clear next steps. Our team lives here, volunteers here, and partners with local hospitals, schools, and community groups so your care stays connected.
Care with us is truly team-based. You’ll have a primary clinician plus a nurse and care coordinator who track labs, follow up after visits, and help with referrals or questions between appointments. Reach us by phone, portal, or video—whatever is easiest—knowing your privacy and preferences are always respected.

A photo ID, insurance card, medication list, and any recent records. Arrive 10 minutes early to complete forms, or fill them out online in advance.

Can I get a same-day appointment?
Yes. We reserve slots daily for urgent needs. You can also book next-day or virtual visits online.

Do you offer telehealth?
Yes. Many follow-ups and minor concerns can be handled by video. Your clinician will advise when an in-person exam is best.

We’re here to help. Call us, book online, or send a secure message through the patient portal. We offer convenient hours, on-site testing, and prompt follow-up. If you have a medical emergency, call 911.
For appointments and general questions, our team answers the phone during business hours and replies to portal messages within one business day. Same-day visits are often available for urgent needs, and many follow-ups can be done by video. You can also request prescription refills and medical records through the patient portal.
We’re easy to visit with on-site parking, wheelchair access, and clear signage. New patients can complete forms online before arriving to save time. Interpretation services are available—please tell us your preference when you book. If you’re unsure which type of visit you need, call and we’ll guide you.
